Rehab Services / Cardiac Rehabilitation Program

Rehabilitation Services located on the second floor of the hospital operates Monday to Friday providing services to both in-patients and out-patients.  Services are provided by registered Physiotherapists, Kinesiologists, and Physiotherapist Assistants.  A range of therapies are available and a treatment plan will be designed for your specific needs after the physiotherapist completes an assessment.  We offer assessment of physical problems of cardiopulmonary, orthopedics and musculoskeletal nature.  If you are in the hospital and are in need of Rehab Services, your physician will order these services and the physiotherapist will visit you in your room to organize the therapy.  A Physician’s referral is required to obtain Rehab Services as an out-patient and referrals will only be accepted if you do not have third party coverage. (I.e. WSIB, MVC, or Extended Benefits)  Out-patient physiotherapy is available by appointment only.

Who we are?

The Cardiac Rehabilitation Program is a collaborative effort between the Hanover & District Hospital, Hanover Family Health Team, Diabetes Grey Bruce and St. Mary’s General Hospital Cardiac Rehabilitation Program.

What is Cardiac Rehabilitation?
The Cardiac Rehabilitation Program is designed to provide education, counselling and tailored exercise program to adult patients who have experienced a cardiac event such as a heart attack or have some form of heart disease.  The program is designed to assist patients improve their heart health and well-being.

Hanover’s Cardiac Rehabilitation is modelled on the evidenced based St. Mary’s “Hearts in Motion” program, allowing patients to receive the same high quality programming without having to travel. The program is provided by   a team of health professionals including a Physician, Nurse Practitioner, Pharmacist, Kinesiologists, Registered Dietitians and a Social Worker. Once your physician makes a referral for you to this program, you will receive assessments by a Nurse Practitioner and a Kinesiologist, along with a stress test.  These assessments enable the team to create tailored exercise sessions and counseling.  You will begin your program with group education sessions on healthy eating, medications, stress and anxiety management, and smoking cessation counselling service.
